2. Filled in application form should be submitted at area Customer Service Centre in whose jurisdiction the telephone. is presently working

3. Please submit the following documents:

a) Application form.

b) Death certificate.

c) No objection from legal heirs.

d) Indemnity Bond, from transferee.

e) Proof of relationship.

f) Duplicate PAF from transferee.
